CPS Parent Survey / CPS Encuesta para padres de familia
Hello Azuela Families, Today, our district is launching the Fall 2021 Survey (cps.edu/familysurvey) - an opportunity for CPS families and stakeholders to share feedback on our plan to bring all students back to school five days a week next fall. This survey will help us understand how families are feeling about the health and safety of their children and how we can support them as the new school year begins. The window for this survey is brief (June 4 - 18). Please fill out the survey before the deadline. Let your voice be heard!! cps.edu/familysurvey If you did not receive the survey by email, please contact your child's school to verify that you are listed as a primary contact, your correct email and phone number are on file, and that you are opted in to receive all communications from the district. Thank you for all you do.
